You can add titles to your character’s name at levels 15 and 25. You can access this option by visiting any of the trainers in Paragon City. Once selected, the titles will be displayed as such: original character name - Character X; after first title selection - The [insert title here] Character X; after second selection - The [insert first title here] [insert second title here] Character X. It’s important to remember that you will not be given the chance to change your character’s title after the second choice is added. However, you can change your first title choice when you receive your second choice at level 25. You can also continue through the game without choosing a title, and the option will still be available to you until you do so. And the word "The" is optional.
